Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees in that employees were exposed to a crushing hazard: (a) LEED Energy Services, at Drill Site WILLSOURCE 1-13, Rifle, CO: On or before September 14, 2004, the employer did not ensure that the wind guy anchors were set according to the manufacturer's specifications, thereby exposing five or more employees to the hazard of being crushed in the event of derrick collapse. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter").
