Employees were allowed to perform steel working operations while standing
on a work platform
that was lifted by a rough terrain forklift. The work platform extended a
total of fourteen (14)
feet beyond the load backrest.

correct this
hazard is to properly construct the work platform in accordance with
Paragraph
8.24.1(b)
of the American Society of Mechanical Engineers (ASME) B56.6-2002, Safety
Standards
for Rough Terrain Forklift Trucks.
