General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not provide employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that employees were exposed to caught-in hazards at the conveyor: a)On or about June 16, 2005, and at times prior, employees were exposed to rotating parts on the feed conveyor when shovelling aggregate and pumice that had fallen from the belt at the load belt located outside the facility. The conveyor was not provided with an emergency stop on the side where the employee was working. Among other methods, one feasible and acceptable abatement method to correct this hazardous condition would be to comply with ANSI B20.1-2003, Section 5.11.2C, and provide emergency stop buttons, pull cords, limit switches or similar emergency stop devices for all remotely and automatically controlled conveyor where operator stations are not manned or are beyond voice or visual contact from drive areas, loading areas, transfer points, and other potentially hazardous locations on the conveyor path not guarded by location, position, or guards. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").
