Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a. Warehouse area: Moveable shelf racks being used in the warehouse for material storage (a) were not anchored to the floor; and (b) did not have plaques identifying the rated load capacities of the racks to verify the maximum amount of material that could be stored safely. Among other methods, one feasible method of abatement is to: (1) anchor all racks securely to the floor; and (2) install plaques indicating the rated load capacities of the racks in accordance with ANSI MH16.2-1984(R1996),"For the Use of Industrial and Commercial Steel Storage Racks - Manual of Safety Practice/A Code of Safety Practices."

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling from defective ladders: (a)Warehouse area: Fiberglass ladders in use had damaged side rails, bent and damaged cross bracing. (b) Warehouse area: Fiberglass ladders were in use that had not been inspected. Among other methods, one feasible method of abatement is to: 1) Provide ladders that meet the requirements of ANSI A14.5, Portable reinforced plastic ladders. 2) Perform inspection and maintenance of ladders, and remove ladders from service when damaged as described in ANSI A14.5, Portable reinforced ladders, section 9.4.
