Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to severe burns from flame explosions while charging the furnace with wet trimmings through the top hatch: (a)Rotary Gas Fired Furnace, Number 2 - Wet trimmings used to charge the rotary gas fired furnace were not preheated or sufficiently dried to eliminate trapped water and moisture. A fireball occurred when wet trimmings from Bosch tank numbers 3 and 4 were charged into the furnace through the top hatch, resulting in an explosion, on or about March 20, 2005. VERIFICATION REQUIRED AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS THE DEVELOPMENT AND IMPLEMENTATION OF A PROCEDURE TO REMOVE SURFACE AND ENTRAINED MOISTURE FROM THE CHARGED MATERIAL AND TOOLS (REF. ANSI STANDARD Z241.2).
