Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling loads and/or falling hoist parts: a) Production Area Bay 54 - On or about November 1, 2005 the #2 work station electric overhead jib hoist had been overloaded causing catastrophic failure. (1) Rated load capacity was not permanently marked on the hoist; (2) Initial load test was not conducted with a written report to confirm the hoist's load rating; (3) Crane scale (Dynamometer) was not used to ensure load capacity was not exceeded; (4) Monthly preventative maintenance (PM) inspection program was not established. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to develop a preventative maintenance program which incorporates all recommendations outlined in the manufacturers manuals with written, signed and dated reports and also in accordance with ANSI B30.16 - Overhead Hoists in order to ensure a safe and proper functioning hoist.
