OOOOOOOOVSection 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to flying fragments in the event of wheel failure: (a) On or about November 5, 2009: on the east side of the building near the foreman's office, the double bench grinder # 468 Model 224258 was not used and maintained in a safe manner. Employees were using the grinder by, among other methods, grinding so as to cause a deep furrows at the center of the wheel. The wheel was in need of proper dressing. (b) On or about November 5, 2009: in the panel shop, bench grinder # 0395 was not used and maintained in a safe manner. Employees were using the grinder by, among other methods, grinding so as to cause a furrow in the center of the wheel. The wheel was in need of proper dressing. Among other methods, one feasible and acceptable means to correct this hazard is to ensure that abrasive wheels and abrasive wheel machinery are used, maintained and stored in accordance with the American National Standards Institute, ANSI. B7. 1-2000 Safety Requirements for the Use, Care and Protection of Abrasive Wheels. Additionally, employees must be trained in the proper use of abrasive wheels, the correct procedures for mounting abrasive wheels and proper maintenance of abrasive wheel machinery.
