29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) PSB Industries, Inc., Production areas, on or about February 11th, 2020 and times prior thereto: Some employees were required to wear powered air purifying respirators (3M Adflo brand) during work tasks including, but not limited to, flux core welding operations. The companys written respiratory protection program did not include site specific information including, but not limited to, the following: 1) Identification of the physician or other licensed health care professional that will conduct medical evaluations; 2) Procedures for selecting respirators for use and the types of respirators used in the workplace. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: a) PSB Industries, Inc., Welding areas, on or about February 11th, 2020 and times prior thereto: Some employees were required to wear powered air purifying respirators (3M Adflo brand) during work tasks including, but not limited to, flux core welding operations. Some employees were not provided with medical evaluations prior to use of powered air purifying respirators. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

The employer did not evaluate the workplace to determine employees exposure levels to welding fumes during welding operations onsite.
