platform that was being used improperly:
On or about 05/26/06, employees were engaged in the task of removing
wooden poles from the
ground while being exposed to falling out from an elevated work platform
that was being used
to pull the poles with a rope.
Among other methods, one feasible and acceptable method to correct this
hazard is for the
employer to prohibit equipment operators to deviate from the manufacturer
recommended
operations.
