On or about May 17, 2013, and at times prior thereto, employees were free rigging beams on the times of a JLG SkyTrak Rough Terrain Forklift without manufacturer's prior written approval. Free rigging from forklift tines creates unstable loads and/or tipping of the forklift and exposes employees to the hazards of being struck by falling loads. Only manufacturer approved or professionally engineered lifting devices are permitted to be used.
