What this inspection record means

OSHA opens inspections for many reasons — routine scheduling under a national or local emphasis program, an employee complaint or referral, or a follow-up after a reported injury. Opening or conducting an inspection is not itself an allegation or a finding that this employer broke any rule; any findings appear as the citations listed below, and citations can be contested, reduced, or withdrawn.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to being struck by rolling logs and/or 3/4 inch
diameter wire rope
from a failure of the carriage haul-back line:
(a)Aden Creek Sale: On or about May 17, 2007, and at time prior thereto,
the
Thunderbird skyline's carriage haul-back line consisted of two sections of
3/4
inch diameter wire rope joined together using a knot similar to a "square
knot". This practice may reduce the wire rope capacity by approximately
50%
and exposed the sheaves to damage, which could damage the wire rope. The
knot also exposed other sections of the wire rope to damage when spooled
together on the drum.
NOTE: Feasible and acceptable abatement methods, among others, to correct
this hazard are
to use only continuous lengths of rope without splice or to join wire rope
according to the
manufacturer's guidelines in order to maintain its rated capacity. A
"long splice" by a
qualified person may meet the manufacturer's guidelines. According to
Idaho Administrative
Code, IDAPA 17.08.09 - Minimum Standards & Practice for Logging - Rigging,
Lines,
Blocks & Shackles: "two lines may be connected by a long splice."
NOTE: Abatement certification IS required for this item.
