Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to potential struck-by hazards resulting from failure of the
homemade "clamps"
and/or" A","C", and "L" hooks used to lift/raise reels of wire with
overload hoists. The hooks
were welded/repaired in house and tests were not made following repair to
ensure the strength
and safety of the hook.
Among other methods, one feasible abatement method would be to ensure the
provisions of
ANSI/ASME (American National Standards Institute/American Society of
Mechanical Engineers)
B30.20 Safety Standard for "Below the Hook Lifting Devices" or its
equivalent are met prior
to the use of lifting devices which have been altered, modified, or
repaired.
