recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to a potential dust explosion hazard that could
have resulted from or
been exacerbated by the failure to locate the dust collector that collects
combustible dust
outside of the building, and by the failure to isolate the dust collector
from upstream and
downstream processes:
(a)  Mixing room:  The Aget Model FT64S101-D1 dust collector, which
collects dusts that are potentially explosive, was located inside the
building, on
or about August 24, 2007.
(b)  Mixing room:  The Aget Model FT64S101-D1 dust collector was not
isolated from other operations by abort dampers and backdraft dampers to
prevent propagation of fires/explosions to or from the dust collector, on
or
about August 24, 2007.
ABATEMENT CERTIFICATION REQUIRED
ABATEMENT NOTE:  Among other methods, one feasible and acceptable abatement
method to correct these hazards includes following NFPA 654 (Standard for
the Prevention
of Fire and Dust Explosions from the Manufacturing, Processing, and
Handling of
Combustible Particulate Solids 2006 Edition) including but not limited to:
Relocate the dust collector outside of the building or comply with the
requirements of the
exceptions listed in paragraph 7.13.1.1.2 of NFPA 654.
Isolate the dust collector from other operations by installing backdraft
dampers or otherwise
meet the provisions of paragraph 7.1.4 (Isolation of Equipment) of NFPA
654.
