North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish each of his employees conditions of employment
and a place of employment free
from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in
that employees were exposed to struck by hazards:
a)establishment - where employee(s) were not provided operating
instruction procedures for
equipment such as, but not limited to, the Core Layer Dryers and the
Surface Layer Dryer
were exposed to explosion hazards. On or about December 2, 2006, an
explosion occurred
when the operator was bringing up the Core Layer Dryer (CL2).
Abatement note:
One feasible and acceptable abatement method, among others, to correct
this hazard is to provide
operating instructions which include, but not limited to, the following:
a) Schematic Piping and Wiring
Diagrams, b) Start-up procedures, c) Shutdown procedures, d) Emergency
procedures
including those
occasioned by loss of special atmosphere, electric power or other
essential utilities, and e) Maintenance
procedures.
