Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to the hazard of getting caught in and crushed by materials or kicker mechanisms on an automatically controlled conveyor: Among other methods, one acceptable and feasible method to abate this hazard would be to install an audible and/or visual warning system that alerts the workers prior to the start up of the conveyor system. Alternatively, another feasible and acceptable method would be to install a barrier, guard or guardrail system to prevent employees from accessing the area.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to: Slips and falls while working/walking on corrugator's exit discharge roller conveyor. Among other methods, a feasible and acceptable method to correct this hazard would be to ensure that tread plates are installed in all locations on conveyors where employees must walk/work and ensure that employees walk only on tread plates and not rollers.
