Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to:

employees were exposed to:
a.At the Insulation Building, full and empty reels were stored across the
aisle
from Plastics Line 6 and in the area between Plastics Lines 5 and 6 and
were
not secured to prevent them from rolling. There were conditions present
such
as forklift traffic and uneven floor surfaces that could cause reels to
roll
where there were employees walking in the area exposing them to being
struck
by a moving reel.
b.At the Insulation Building, after removing a full reel from the take ups
on
Plastics Lines 5 and 6 employees did not always secure the reels to prevent
them from rolling.  There were conditions present such as forklift traffic
and
uneven floor surfaces that could cause reels to roll   where there were
employees walking in the area exposing them to being struck by a moving
reel.
Feasible and acceptable abatement methods could include but are not
limited to:
1.Provide an appropriate method to secure the reels in storage such as a
raised
area on either side of the reels to prevent them from rolling (a  metal
plate
secured to the floor with a raised area or lip on either side of the reel)
2.Remove reels from the take up using a forklift and immediately transport
them
to a storage area where they are secured to prevent them from rolling.
3.Provide an appropriate method to secure the reels after removing the
reels
from the take ups such as chocks.
4.Provide  chocks that  are large enough to prevent the reels from rolling
over
them.
5.Provide training to employees that includes methods of chocking reels so
that
the employees do not place their body or body parts under the reel or in a
position where the reel could roll over them.
6.Require employees to secure the reels to prevent them from rolling and
enforce the policy.
