1970: The employer
did not furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard(s) of:
a)  Plantwide - On or about June 10, 2009, employees were exposed to being
struck-by or
punctured by high velocity projectiles during the rupture of polyvinyl
chloride (PVC) pipe with
a line pressure of 90 pounds per square inch (psi) being used to transport
compressed air to
operate the gluers, printing presses, sample and bander machines. The
piping was not approved
for use with compressed air.  Fragmentation of the pipe can occur because
of fatigue caused by
cold contents and/or vibrations or the compressed shifting position and
placing additional stress
on the PVC pipe connector.
Among other methods, one feasible and acceptable means of abatement is to
ensure that PVC
pipes under pressure are approved for use with compressed air and are
constructed of or encased
in a shatter resistant material per the American National Standards
Institute/American Society
of Mechanical Engineers (ANSI/ASME) B31.8-1986, Gas Transmission and
Distribution Piping
Systems Standard, sections 842.32, 842.43 and 849.52(b), the American
Society for Testing
Materials (ASTM) D1785-86, Standard Specification for Polyvinyl Chloride
Plastic Pipe,
Schedules 40, 80 and 120 and ASTM D2513-86a, Standard Specification for
Thermoplastic Gas
Pressure Piping Systems.
