Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck-by hazards, while operating a wheeled mobile crane:
On or about 02/08/08 at McDonough, GA: Employees were exposed to struck-by
hazards, while
performing work with a wheel mounted mobile crane. The following hazards
were identified
during the inspection of the mobile crane's hook:
1. The employer did not perform frequent and regular inspections on the
mobile crane's hook,
which was used in the lifting process.
2. The mobile crane's hook did not have a safety latch installed.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS:
1. The employer shall ensure that frequent and regular inspections are
performed on the wheeled
mobile crane's hook. ANSI B30.10-1993, Paragraph 10-1.2.1.1, states that
hooks in "normal"
service should be inspected monthly and receive a yearly documented
inspection. Pre-operational
inspections of the hook should also be performed.
2. The employer shall ensure that hooks are equipped with a safety latch
that closes the throat
opening.  Defective hooks shall be removed from service until the latch is
replaced or repaired.

In accordance with 29 CFR 1903.19(c), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

crane identified the
following hazard:
1. The wheel mounted mobile crane had a telescopic boom, which did not
have an anti
two-block device installed.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS:
1. The employer should ensure that an anti two-block device is installed
on the boom of
the crane to prevent contact between the hook assembly and the boom point
sheave assembly.
