Carolina:  The employer did not furnish employment and a place of
employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to fire and explosion hazards:
a)facility - board line waste, combustible dust accumulated up to 1-inch
thick on
horizonal surfaces including piping and electrical equipment and was
clinging to
vertical surfaces such as walls, machinery and electrical equipment due to
failure
to control fugitive emissions and poor housekeeping.
b)facility - extruder lines 11-15, combustible dust accumulated
approximately 1/4-
inch thick on horizonal surfaces including piping, ductwork, I-beams, and
electrical equipment such as the top of lighting fixtures and disconnects
due to
inadequate housekeeping.
One feasible and acceptable method of abatement, among others, to correct
this hazard
is the employer to follow the procedures in the NFPA document 654,
Standard for the
Prevention of Fire and Dust Explosions from Manufacturing, Processing, and
Handling
of Combustible Particulate Solids.
