29 CFR 5(a)(1):  of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment which
were
free from recognized hazards that were causing or likely to cause death or
serious
physical harm to employees in that employees are exposed to severe burns
from
potential dust explosion, deflagration and fire hazards caused by the lack
of a cleaning
schedule of the plant:
Throughout the facility located at 4400 E. Highway 30, Kearney, NE - The
employer
is failing to ensure that employees working in the facility are protected
from explosion
and fire hazards associated with excessive dust accumulations in the
facility.
Throughout the facility combustible paper dust has been allowed to build
up in excess
of 1/8" on the HVAC and piping systems near the ceiling of this area.
This issue
exists in areas including, but are not limited to the microlyte area.
Among other methods, feasible and acceptable methods of abatement are:
1)  Ensure that the whole facility is cleaned to remove the excess dust
accumulations.
2)  Develop and implement an inspection protocol pertaining to the overhead
HVAC and piping systems to identify areas of dust build.
3)  Develop and implement a cleaning schedule to address the whole facility
with special emphasis on areas which have proven to be high accumulation
areas.
Abatement certification and abatement documentation is required for this
violation.
The documentation should include written verification of abatement,
applicable
measurements or monitoring results, and photographs or videos which you
believe
will be helpful. The abatement certification sheet is enclosed with the
citations.ion
