Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential injuries resulting from fire and explosion: a) Kuehne Chemical Company, Inc., 1645 River Road, Delaware City, DE 19706 - On about July 10, 2010 three employees were seriously burned when they attempted to forcibly remove a jammed roller on the salt belt recovery system resulting in the ignition of friction sensitive chlorate crystals and finely divided titanium metal, a reactive mixture that had accumulated in and around the shaft end. ABATEMENT NOTE: Feasible methods of abatement among others include: 1) Install a reliable automatic water rinsing system at the collar and shaft connections to constantly remove finely divided titanium metal and any salts that could ultimately form chlorates. Include this system on a regular maintenance schedule as needed; 2) Schedule and document routine maintenance to manually hose down the salt belt recovery system to prevent significant accumulations of chlorates on all parts and finely divided titanium metal in and around the titanium shafts and collars; 3) Produce an alternative salt belt recovery system to preclude the generation of flammable finely divided compounds such as, but not limited to, titanium metal, consisting of shafts and collars constructed of alternative materials.
