"Section 5(a)(1) of the OSH ACT of 1970:The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fire hazard: Hydrogen Compression System - Employees are exposed to fire hazards from the use and failure of fiberglass reinforced plastics piping in the hydrogen vent system that is not approved for the service. Condition identified on June 10, 2014. One feasible and acceptable abatement method, among others, is to comply with the sections of CGA G-5.5, Hydrogen Vent Systems, applicable to your system and ensure qualified personnel who are competent and experienced in the design and design codes applicable to hydrogen pressure relief systems review and approve the pressure relief valve, vent sizing, analyzer and flame arrestor vent system."

29 CFR 1910.119(j)(6)(iii): The employer did not assure that maintenance materials, spare parts and equipment are suitable for the process application for which they will be used: a) Hydrogen Vent (Roof Line) and Chlorine System Header - Employees were exposed to a fire and a toxic chemical inhalation hazard when a "dry" liquid chlorine header pipe connection to a pressure gauge with a housing constructed of incompatible titanium metal, failed releasing chlorine gas that subsequently reacted exothermically with hydrogen gas emission from an overhead hydrogen vent. Condition reported on June 10, 2014.
