Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish a place of employment which was free from recognized hazards that
were causing or
likely to cause death or serious physical harm to employees in that
employees were exposed
to struck by hazards:
(a)Xtreme Coil Drilling Corporation at Rig #11 Waterfront #1-3, Longmont,
CO
80504:  On or before 8/25/08, the employer did not ensure the wire rope
slip and cut
practices were followed in accordance with the wire rope manufacturer's
directions.
The Union Wire Rope Cut-off Program provided that the employer was not to
accumulate more than 1300 ton miles between cuts.  The employer's ton mile
log
showed that the accumulated ton miles since the previous cut exceeded 1300
ton miles
in the following instances:
1)On 4/13/08, 2,918 ton miles were recorded on the wire rope since the
previous cut; and
2)On 6/6/08, 2,141 ton miles were recorded on the wire rope since the
previous
cut.
In addition, the manufacturer provided that the employer was to keep the
History
Sheets current, accurate, and complete.  However, the employer did not
record and
update the accumulated ton miles on the wire rope since May 23, 2008,
three months
prior to the OSHA inspection.  Conditions exposed employees to struck by
hazards if
the wire rope was to part.
Abatement Note:  Abatement certification is required for this item (See
enclosed "Sample
Abatement-Certification Letter").
Among other methods, one feasible and acceptable abatement method is
stated in API
Recommended Practice 54, Section 9.6.6, which provides that, "When
calculations indicate
ton mile limits have been reached, or visual inspection shows breaks,
crushing, or damage,
the wire rope shall be slipped, cut, or replaced."
