Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards:
(a)On April 30, 2008 at 614 Jonathan, Marionville, Missouri - Employees
were
exposed to a fall hazard while working on a platform that was not capable
of
supporting at least four (4) times the maximum intended load in accordance
with
ANSI B56.1-1969.  The basket extended four (4) feet beyond the wheel base
of
the forklift.
Abatement certification is required for this item - use the CERTIFICATION
OF
CORRECTIVE ACTION WORKSHEET for this purpose, and in addition,
documentation
demonstrating that abatement is complete must be included with your
certification.  This
documentation may include, but is not limited to, evidence of the purchase
or repair of the
equipment, photographic or video evidence of abatement, or other written
records.
