Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by the moving robot: a)Injection Molding Dept., on or about 8/12/09: The barrier guard for the robot on the Van Dorn #10 injection molding machine had the interlocks bypassed with washers and tape, thus the doors could be opened with the robot in motion. Also, the barrier guards provided did not prevent entry into the area of the moving robot arm around the conveyor in that the guards needed to be extended at the top and bottom on three sides. b)Injection Molding Dept., on or about 8/12/09: The moving robot arm was not adequately guarded on the Nissei #17 injection molding machine. c)Injection Molding Dept., on or about 8/18/09: The barrier guards for the robots on the injection molding machines Sumitomo #9, Nissei #11, #12, 13 and #15 were not interlocked. Among other methods, one feasible and acceptable method to abate these hazards are to: 1.Install interlocks on the barrier guards that stop the robot and remove drive power to the robot actuators and/or stop automatic operation of the robot and any other associated equipment that may cause a hazard in accordance with ANSI R15.06-1992, Section 6.3.3. 2.Remove the bypass devices such as washers and tape so that the interlock guards operate properly. Abatement certification must be submitted for this item.
