29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:   a) On or about 2/25/2014,  Blow Molding Area:  Employees of a contractor in the process of removing a blow molding head in conjunction with Currier Plastic employees was not utilizing lock out.  The hoist employer, Currier plastics, had not informed the contractor of the requirements of the lock out program and of the equipment  lock out procedures.     Abatement certification is required for this item.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by moving parts:   a) On or about 2/25/2014,  Blow Molding Area:  The exit conveyor on a Model A700 blow mold machine created a pinch point between the conveyor and the frame.     b) On or about 2/25/2014, Blow Molding Area:  A series of plexi glass guards were removed from the #1 Heins blow mold machine.     c) On or about 2/25/2014, Blow Molding Area:  Two out feed conveyors on the Automa A4002 blow mold machine had pinch points between the belt and metal sorter slides.     d) On or about 2/25/2014, Blow Molding Area:  Two out feed conveyors on the Automa A4001 blow mold machine had pinch points between the belt and the metal sorter slides.   Abatement documentaion is required for this item.  Currier Plastics Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 312370745, citation number 1, item number 5 and was affirmed as a final order on 3/3/2010, with respect to a workplace located at 101 Columbus Street, Auburn, NY.
