recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to potential fire and explosion hazards because methods,
practices and equipment
were not utilized to prevent propagation of a fire and/or explosion into
employee occupied areas:
(a)Pole Barn Grinding Area - The Aluminum grinding dust collection system
was
not electrically grounded and bonded, thereby exposing the employees to a
fire/explosion hazard, on or about June 3, 2009.
(b)Pole Barn Grinding Area - The Aluminum grinding dust ventilation system
was
not equipped with deflagration venting, thereby exposing the employees to a
fire/explosion hazard, on or about June 3, 2009.
(c)Pole Barn Grinding Area - The Aluminum grinding dust collectors were
not of
the electrostatic type, thereby exposing the employees to a fire/explosion
hazard,
on or about June 3, 2009.
(d)Pole Barn Grinding Area - The Aluminum grinding dust collector was not
emptied at least daily, thereby exposing the employees to a fire/explosion
hazard,
on or about June 3, 2009.
(e)Pole Barn Grinding Area - The Aluminum grinding dust collection system
was
not effective in that there were fugitive dusts present in the work area,
thereby
exposing employees to a fire/explosion hazard, on or about June 3, 2009.
(f)Pole Barn Grinding Area - The dust collection hoods and ductwork for the
Aluminum grinding dust were not cleaned regularly to remove the lint and
Aluminum fines, thereby exposing the employees to a fire/explosion hazard,
on
or about June 3, 2009.
(g)Pole Barn Grinding Area - The shovels used to clean up the fugitive
Aluminum
grinding dust were not constructed of electrically conductive, non-sparking
material, thereby exposing the employees to a fire/explosion hazard, on or
about
June 3, 2009.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable method to
correct these methods is to comply with National Fire Protection
Association (NFPA)
484 Standards for Combustible Metals (2006 Edition) as it applies to your
operations and
equipment but not limited to:
*Electrically ground and bond all parts of the ventilation system as
required
in paragraph 6.3.8.2.1.
*Install deflagration venting as required in paragraph 6.3.5.6.
*Install electrostatic type dust collectors as required in paragraph
6.3.5.1.
*Install a measuring device or implement a formal testing program to
ensure the duct velocity is maintained at 4,500 feet per minute or greater
as required by paragraph 6.3.3.2.
*Implement a cleaning program to ensure that the dust collectors are
emptied daily as required in paragraph 6.3.5.5
*Implement a cleaning program for the inside of hoods and ductwork which
is frequent enough to maintain the lint and aluminum fine amounts to a
safe level as required by paragraph 6.3.5.8.
*Implement a testing program to ensure that the ventilation system is
effective in the collection of all Aluminum dust as required in paragraph
6.4.2.1.
*Install warning signs for the Aluminum Grinding Area to warn of the
potential combustible dust hazards as required in paragraph 6.3.2.5.3.
*Install warning tags on each of the grinders used in the Aluminum
Grinding Area to ensure that the equipment is only used for grinding
Aluminum as required in paragraph 6.3.2.6.1.
*Provide a shovel or dust pan made of conductive, spark resistant material
as required by paragraph 6.5.7.7.3.
*Provide employee work uniforms made of flame retardant, non-static
generating material which are designed to be easily removed as required
by paragraph 6.6.2.1.
*Provide static dissipating shoes/boots for the employees in the Aluminum
Grinding Area as required in paragraph 6.6.2.3.
