The employees used a Salisbury voltage detector attached to the end of a hot stick to test for the absence of voltage. This tester is a proximity tester and is not the proper test instrument to be used for this purpose. b) On or about 5/16/09, in the Vault Room section of Electrical Room 18 in the basement of Baldy Hall, SUNY & Buffalo, Amherst Campus and continuing: Employees were scheduled to perform preventive maintenance on load switches & transformers. Prior to performing this maintenance, an employee donned PPE, entered the transformers, tested for absence of voltage, and applied protective grounds on the primary side of transformers T1 & T2. The employee used a Salisbury voltage detector attached to the end of a hot stick to test for the absence of voltage. This tester is a proximity tester and is not the proper test instrument to be used for this purpose. c) On or about 5/16/09, in the Vault Room section of Electrical Room 18 in the basement of Baldy Hall, SUNY & Buffalo, Amherst Campus and continuing: Employees were scheduled to perform preventive maintenance on load switches & transformers. Prior to performing this maintenance, an employee donned PPE, entered load switches LSPS3-53A-SS2 and LSPS2-53A-SS2, tested for absence of voltage, and applied protective grounds on all 3 phases of the line and load sides of the switches. The employee used a Salisbury voltage detector attached to the end of a hot stick to test for the absence of voltage. This tester is a proximity tester and is not the proper test instrument to be used for this purpose. Among others, a feasible method to eliminate or materially reduce this hazard is to use a voltmeter rated for the application to test for the absence of voltage. ABATEMENT DOCUMENTATION REQUIRED
