Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the load:
a) Hand layout area- On or about 10/26/10- The Below-the-hook lifting
device used to
lift Resin molds had no rated load capacity markings or manufactures
markings.
Among other methods, feasible and acceptable abatement methods to correct
the hazard
are to:
1.) Inspect and test the lifting device in accordance with ASME
B30.20-1993, Section 20-
1.3, Section 20-1.4 and Section 20.4.3.
2.) Mark the rated load capacity on the device along with manufactures
name, serial
number and weigh, for the device over 100 lbs in accordance with ASME
B-30.20-1993,
Section 20-1.2.

employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being crushed-by:
a) Hand layout area- On or about 10/26/10- An overhead carrier beam rail
in house
fabricated and welded that was used to lift molds containing resin based
materials
weighing approximately 3,000 lbs was not tested, rated or inspected.
Among other methods, several feasible and acceptable abatement methods to
correct this
hazard are to:
1. Mark the load capacity on the metal beam rail in accordance with ANSI
B30.11-1980,
Monorail Systems and Under Hung Cranes, Section 11.1 Construction and
Installation.
2.) Inspect and test the metal beam rail in accordance with ANSI
B30.11-1980, Monorail
Systems and Under Hung Cranes, Section 11.2 Inspection, Testing, and
Maintenance.

employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that: the
employees were exposed to explosion and deflagration hazards.
a) Trim Booth Ventilation- On and before 1/11/11- Exhaust ventilation to
capture
Hazardous Air Particles (HAP) which was explosive in nature was
insufficient. The
indoor dust collector was not adequately sized, was not equipped with
explosion
protection with deflagration venting to the outside.
Among other methods, feasible and acceptable methods to correct these
hazards are:
1.) Have a qualified engineer inspect the dust collectors to evaluate the
design and installation
in accordance with National Fire Protection (NFPA) 654, Prevention of Fire
and Dust
Explosions from the Manufacturing, Processing, and Handling of Combustible
Particulate Solids
and make necessary modifications.
2.) Equip the dust collection system with explosion proof venting to the
outside of the building.
ABATEMENT DOCUMENTATION AND CERTIFICATION IS REQUIRED FOR THIS

Chapter 6 Facility and Systems Design
6.1 General. The provisions of this section shall apply to the overall
design of systems that
handle combustible particulate solids.
6.1.1* Those portions of the process and facility where a dust
deflagration hazard exists shall
be protected from the effects of dust deflagrations in accordance with
Sections 6.2, 6.3, and
6.4
Chapter 7 Process Equipment
7.1* General. Methods of fire and explosion protection for specific
equipment shall be in
accordance with this section.
Chapter 8.1.1 requires "Continuous suction to minimize the escape of dust
shall be provided for
processes where combustible dust is liberated in normal operation". The
existing dust collection
system actually maximizes the escape of dust throughout the space and Dust
Chamber. The dust
should be collected closer to the point of generation.
Chapter 8.1.2 requires "dust shall be conveyed to dust collectors". The
employer is actually
using the Trimming Booth itself to convey the dust to the Dust Chamber
collector. The room
cannot be used as a method of conveyance.
The indoor dust collector system was not adequately sized, equipped with
deflagration protection
and was not vented to the outside.
