OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: employees were exposed to explosion and deflagration hazards. a) Trim Booth- On and before 02/12/2013- The dust collector system was not adequately designed, installed and maintained to protect employees from fire and explosion hazards associated with accumulation of combustible dust. Hazardous accumulations of combustible dust were identified in and around the trim booth and the associated ventilation system. Solve Composites, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: the employees were exposed to explosion and deflagration hazards, which was contained in OSHA inspection number 313761355, citation number 1, item number 3 and was affirmed as a final order on 05/09/2012, with respect to a workplace located at 29 Distribution Way Plattsburgh, NY 12901. Among other methods, a feasible and acceptable method to correct this hazard is to ensure that the trim booth and associated dust collection systems comply with applicable requirements of NFPA 654 -- Standard for the Prevention of Fire and Dust Explosions from the Manufacturing, Processing, and Handling of Combustible Particulate Solids, 2006 Edition, and ensure that the following requirements are met:

29 CFR 1910.106(h)(4)(i)(d): The storage of flammable or combustible liquids in containers shall be in accordance with the applicable provisions of paragraph (d) of this section. a) Flammable Storage Area - on or about 02/11/13 - Storage containment room containing approximately 1780 gallons of Category 2 and Category 3 flammable liquids stored in (55 gal.) fifty gallon drums and (5 gal.) five gallon pails, including but not limited to: Acetone, Styrene, K133, Poly Lite, Poly Core Gel Coat, Sirpol General Purpose Resin, Gray Gel Coat, Air Dry Mask Gray was not designed in accordance with 1910.106(d)(4) (iv), in that: the storage room was not equipped with either a gravity or mechanical ventilation system to prevent the accumulation of flammable vapors and/or migration of flammable vapors to ignition sources. Where the metallic floorplate on which the container stands while filling is electrically connected to the fill stem or where the fill stem is bonded to the container during filling operations by means of a bond wire, the provisions of this section shall be deemed to have been complied with. a) Reclaim area between mixing room and gel coat paint booth - On and before 2/11/13 - Emloyees did not interconnect the nozzle of a barrel containing acetone, a Category 2 flammable liquid, when open pouring into a metal parts wash bucket.
