that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
Employees riding on and dispensing gravel from the GAR-BRO Rock Hopper,
while
it was being raised and/or lowered by the Caterpillar 325C LCR Hydraulic
Excavator
to the bottom of the excavation were exposed to pinch point hazards, falls
of up to
approximately 10 feet and to the hazard of being caught between the
sidewalls of the
excavation and the hopper.
Feasible and/or useful abatement methods to correct the hazards include,
but are not
limited to the following:
1)Establish a policy that: prohibits riding rock hoppers, requires
communication
of it to employees, and enforcement.
2)Adhere to the manufacturers warning sticker located on the outside of the
equipment.
