29 CFR 1910.119(j)(6)(iii): The employer did not assure that maintenance materials, spare parts and equipment were suitable for the process application for which they were used: a) Jobsite: Philadelphia Refinery, Hydrofluoric Acid Alkylation Unit, Depropanizer - The employer did not assure maintenance materials, spare parts and equipment were suitable for the process application for which they were used. A heat exchanger tube repair plug that failed was used in a service that exceeded its allowable service pressure. ASME PCC-2, Repair of Pressure Equipment and Piping, 2008 Edition, specifies a maximum service pressure for tapered plugs to be 200 psi. The maximum allowable working pressure for the shell of the exchanger and the repair was 350 psi. NOTE: ABATEMENT DOCUMENTATION REQUIREDATION
