South in Wilson, Arkansas, southeast corner of the plant, the 1959 #1487
Freight
Elevator that was being used by employees to transport materials between
the 1st
and 2nd floor levels was equipped with an approximately 51" high gate that
did
not extend the approximately 6'8" height of the elevator car.  The gate
did not
prohibit employees from having any part of their body outside the elevator
gate
while the elevator was in operation. This condition exposed employees to
struck
by and caught between hazards.

Elevator
that was being used by employees to transport materials between the 1st
and 2nd
floor levels.  Employer allowed employees to use the elevator with an
expired
Elevator Permit dated 11/2007.  Employees were exposed to fall, struck by
or
crushed by hazards.
Feasible and acceptable methods to correct this hazard is:
(A)Ensure the elevator is inspected, tested by a licensed authority and
is maintained in accordance with ASME A.17.1 Safety Code for
Elevators and Escalators.
(B)Incorporate specific training procedures relevant to elevator safety
into your current documented safety and training program.
