Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the load should the overhead
hoist, below-the-
hook lifting device or bridge rail fail:
a)Bindery, on or about 8/25/10:  A 1/4 ton Coffing overhead hoist at the
Hamilton #1 collator had no safety latches at the lifting hook and at the
hook
that attaches the hoist to the trolley and a one ton overhead hand chain
hoist
at the Form Flo continuous collator had a broken safety latch at the
lifting
hook and no safety latch where the hook attaches the hoist to the trolley.
Also, no inspections were conducted on the overhead hoists.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Install safety latches on the hooks in accordance with ANSI B30.16-1993,
Section 16-1.2.9.
2.Conduct frequent and periodic inspections of the overhead hoists in
accordance with ANSI B30.16-1993, Section 16-2.1.
b)Bindery, on or about 8/25/10:  A below-the-hook lifting device used for
lifting
rolls of paper at the Hamilton #1 collator had no rated load capacity
markings on the device and had not been inspected.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Mark the rated load of the lifting device on the main structure or
equivalent
in accordance with ASME B30.20-1993, Section 20-1.2.1.
2.Inspect the device in accordance with ASME B30.20-1993, Section 20-1.3.
c)Bindery, on or about 8/25/10:  A bridge rail at the Form Flo continuous
collator with a one ton overhead hand chain hoist attached and a bridge
rail
at the Hamilton #1 collator with a 1/4 ton Coffing overhead hoist attached
did not have rated load markings on them.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to:
1.Mark the rated load markings on the bridge rail.
Abatement certification must be submitted for this item.
