(RDS) was not protected from falls in that the safety line and the primary
working line were attached to the same anchorage point.

AL, the primary working line and lifeline were not attached within 15
degrees of
perpendicular of the area being accessed.
Among other methods, one feasible and acceptable method to correct this
hazard is to follow the
guidelines of the American National Standards Institute/International
Window Clean Association,
ANSI/IWCA I-14.1-2001, Section 5.7.17  Suspension Devices for Rope Descent
Systems and
Section 14.6.2  Roof Anchorages.
