ETBE unit; the startup procedures for furnaces including, but not limited to, furnace F6101 did not address control board operator verification of pilot on the DSC prior to opening main gas valve. This condition exposed employees to explosion and fire hazards. 2.ETBE unit; the employer did not specify in the startup procedures who was qualified and authorized to relight the furnaces including, but not limited to, furnace F6101.
