29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment: a) Range Line- The employer did not ensure that employees utilized protective eye equipment while performing assigned operator duties on the # 6 range line. Employees were exposed to the hazard of ammonia, hydrogen peroxide and THPS while performing assigned duties for the manufacture of fire retardant fabrics. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Ammonia Storage- The employer did not ensure that an eyewash/shower unit was maintained and/or strategically positioned in areas where an exposure to ammonia may occur. No eyewash/shower facility was available at the ammonia unloading area for employees performing servicing and maintenance of the ammonia storage tank. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
