29 CFR 1910.119(d)(2)(i)(A): The process safety information (PSI) pertaining to the technology of the process did not include a block flow diagram that contained accurate information: a) Finishing Department: The employer's PSI (Process Safety Information) failed to contain a block flow diagram for the ammonia process that indicated accurate information regarding process equipment in relation to the flow of ammonia. The # 4 FR range was included in the flow of ammonia in the process but was not active nor involved with the current ammonia process. Employees were exposed to the hazards of ammonia while performing maintenance, repairs and other operations involving the manufacture of flame retardant fabric.

29 CFR 1910.119(m)(1): The employer did not investigate each incident which resulted in, or could reasonably have resulted in a catastrophic release of highly hazardous chemical in the workplace. Mount Vernon Mills-Finishing Department: The employer failed to investigate and/or develop a written incident report of a near miss that may or could have resulted in an catastrophic occurrence when elevated spikes in the ammonia detection system exceeded the emergency evacuation setpoint of 150 PPM. Employees were exposed to the hazards associated with ammonia while performing maintenance, repairs, FR operations and other assigned duties involving the manufacture of flame retardant fabric. a) On or about April 07, 2014 through April 09, 2014 elevated ammonia levels were detected in the FR-6 ammonia range(penthouse) up to 191 PPM. b) On about April 07, 2014 elevated ammonia levels were detected at the FR-7 ammonia range (penthouse) in excess of 257 PPM.
