HAZARD: BURNS Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential explosion and fire hazards from a dust collection system located inside the building and collecting combustible dust: a) Regrind (grinding area) - On or about December 22, 2011, a dust collection system with a CP Environmental, 220 cubic foot model 84-NFB 064 C-M-G baghouse, was installed and used inside the building without devices to prevent deflagration propagation in the ductwork or explosion venting to a safe area of the building exterior. Among other methods, feasible means to abate the hazards would be to: 1. Locate the dust collection equipment outside the building 2. Ensure the dust collection system complies with generally recognized standards and guidelines such as but not limited to: NFPA 68 - Guide for venting of deflagrations NFPA 69 - Standard on Explosion Prevention Systems NFPA 654 - Standard for the Prevention of Fire and Dust Explosions from the Manufacturing, Processing, and Handling of Combustible Particulate Solids Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
