Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing, or likely to cause, death or serious physical harm to
employees in that employees
were exposed to fall hazards from a vehicle-mounted aerial ladder:
On October 25, 2010, and times prior thereto, employees worked from a
personnel basket of a TopKick Diesel Power Ladder Truck, ID number TDLR
18035 at this job site. Employees were exposed to fall hazards as a result
of
incorrectly maintained equipment such as, but not limited to:
A.The 5/8 inch wire rope that raised and lowered the
personnel basket had broken wire strands in several spots
and was also displaying visible rust and corrosion on other
parts of the wire rope. Employees were exposed to fall
hazards in the event of a break of the wire rope.
B.Electrical wiring of the aerial ladder was deteriorated at
numerous places and also showed signs of a lack of strain
relief at connection points.
Among other methods, one feasible and acceptable abatement method to
correct
these hazards is to perform maintenance and conduct and record inspections
and
tests on the vehicle-mounted elevating and rotating aerial ladder in
accordance
with American National Standard Institute, Vehicle-Mounted Elevating and
Rotating Aerial Devices, Standard A92.2-1990, Section 8 for owners and
users,
as stipulated below:
1. Perform inspections of mobile units at designated intervals for the
following
items:
A. Parts, such as pins, bearings, shafts, gears, rollers, locking devices,
chains,
chain sprockets, wire ropes, and sheaves for wear, cracks or distortion.
B. Electrical systems and components for deterioration or wear including
those
not readily visible on a frequent inspection.
AAA Electrical Signs was previously cited for a violation of this
Occupational Safety and Health
Standard which was contained in OSHA Inspection Number 314454778, Citation
Number 01,
Item Number 001, and was affirmed as a final order on 10/19/2010, with
respect to a workplace
located at 2407 E Business Hwy 83, Donna, TX 78537.
