29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: An employee working from a personnel platform mounted on a Terex crane, model BT-3470, was not connected to the boom or basket by a lanyard attached to a harness or body belt. The employee was exposed to falls from heights up to approximately 45 feet.
