29 CFR 1910.180(d)(4)(iv): Worn, cracked, or distorted parts such as pins, bearings, shafts, gears, rollers and locking devices. On June 23, 2016, the employer provided a working platform with a locking pin mechanism that had been cracked/ damaged as to degrade the structural competence of the mechanism as it attaches to the truck crane. Employees were working from the bucket platform, providing maintenance to a business sign approximately 20 feet above ground.

29 CFR 1910.333(a)(1): Live parts to which an employee may be exposed shall be de-energized before the employee works on or near them, unless the employer can demonstrate that de-energizing introduces additional or increased hazards or is infeasible due to equipment design or operational limitations. Live parts that operate at less than 50 volts to ground need not be de-energized if there will be no increased exposure to electrical burns or to explosion due to electric arcs. On June 23, 2016, the employer did not establish safety-related work practices consisting of de-energizing live parts to prevent electrical shock and or other injury resulting from direct electrical contact when performing work on circuits which were or could be energized.

The "U" bolt on the Crosby clip was missing a nut and was ineffective in securing the load. Employees have potential for exposure to struck-by hazards as a result of an improperly secured load.
