furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees
in that employees
were exposed to fall hazards while riding in an unrated site built basket,
that is lifted by an
underhung hoist on a beam:
a)5301 Highway 43, Satsuma, AL 36572; On or about November 15, 2010 and at
times prior thereto employees were exposed to fall hazards while riding in
an
unrated site built basket that is raised and lowered by an underhung hoist
on a
beam used for parts storage and retrieval from a rack system. The employer
failed to follow the American National Standard Institute for Monorail
Systems
and Underhung Cranes (ANSI B30.11 - 1973) Section 11-3.2.3 Moving the Load,
Item e. The operator shall not hoist, lower or travel while anyone is on
the load
or hook. A feasible and acceptable method to correct this hazard is to
install a
ladder system or redesign the rack system so that the parts can be
accessed from
the ground level.
