a) Main Building, aisle 36, rack 3662: Employees of Bradley Caldwell, Inc.
were using a
damaged industrial storage rack to store merchandise.  The front left
column was bent and not
bolted to the floor.  The employees were exposed to struck-by and/or
crushed-by hazards created
by the use of a damaged industrial storage rack.  Observed on or about May
19, 2011.
Among other methods, one feasible and acceptable method to abate this
hazard is to repair
damaged racks and ensure all rack columns are bolted to the floor in
accordance with ANSI
MH16.1:2008, Section 1.4.9 Rack Damage.  Repairs to the damaged industrial
storage racks
must be approved by the rack manufacturer or qualified person.
