29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. a) The product stored on three and four tier storage pallet rack system(s) located on rows eight(#8) and nine(#9) had damage to the horizontal structural members that could effect the strutural integrity and weight capacity of the racking system and having the potential for cllapse of stored material.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5). a) The employer did not ensure that the operators of an electric powered pallet jacks were trained prior to allowing them to operate a powered industrial lift.
