were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck-by and crush hazards:
a)At KFH Industries:  On or about October 5, 2011 and at times prior the
employer
failed to require and enforce the use of seat belts for all employees
operating
forklifts on site exposing them to struck-by and  crush hazards from being
thrown
from the lift.
Among other methods, one feasible and acceptable method to correct this
hazard is to follow the
instruction in the operations manual which states "Always make sure your
seatbelt is securely
fastened".
