(b) In areas throughout the plant: On September 29, 2015 and at times prior to; the employer exposed his employees to slip hazards in that employees were required to walk and work around standing water created by leaks in the roof after a rain.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) At KFH Industries: On September 29, 2015 and at times prior to; the employer exposed his employees that operate and work around forklifts to struck-by hazards in that all forklift operators were not trained or evaluated by the company prior to being required to operate forklifts. KFH Industries, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(i), which was contained in OSHA inspection number 315846295, citation number 1, item number 4, and was affirmed as a final order on November 22, 2011, with respect to a workplace located at 2926 Columbia Hwy, Dothan, AL.
