29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers: On or about June 1, 2011, at the above addressed jobsite, employees conducting carpentry operations on a bridge were exposed to injuries from stepping into holes measuring sixteen (16") inches in depth by thirty-four (34") inhes width.
