OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an amputation hazard from the unintended activation of press brakes. On or about April 27, 2011, Press Brake #1, Chicago Dreis & Krump, Press Brake #2, Milwaukee, Press Brake #3, Di-Acro, Press Brake #4, Di-Acro and Press Brake #5 did not have the foot pedals guarded/covered to prevent unintended activation of the press brake. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
