29 CFR 1910.215(b)(9):     The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    In the maintenance area, the Jet 10" bench grinder's peripheral member at the top (guard) exceeded 1/4 inch as it was 1 3/4 inches on the right side and 1/2 inch on the left side.    No abatement certification or documentation is required for this item.

The  certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection and the person performing the inspection. 29 CFR 1910.1200(f)(5):     The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein and an appropriate hazard warning:    a.  In the Fiber Cutting Department, the 5 gallon can of Aromatic 100 Solvent did not have an appropriate hazard warning listed.    b.  In the Fiber Cutting Department, the 5 gallon can of oil was not labeled with the identity of the oil nor the appripriate hazard warning.

29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.     On or about June 9, 2011, in the Cure Plant, a chain guard rail had been removed in order to place a pallet of material on the platform for the Cure Mix Tanks and it was not replaced when the pallet was removed.    No abatement certification or documentation is required for this item.

29 CFR 1910.213(h)(1):     On radial arm saws, the sides of the lower exposed portion of the blade was not guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contactwith stock being cut to give maximum protection possible for the operation being performed.    In the Fiber Cutting Department, the lower portion of the blade, on the 10" Craftsman Radial Arm Saw, was not guarded.    No abatement documentation or certification is required for this item.
